The prayer at the bottom of p. 456 is excellent.  The prayer at the top of the page is reserved for the priest as involves santificaton); prayers on page 460 for physicians and nurses is fine to use if the beginning is adapted (We pray, O Lord, for those whom you have called....).

Eucharistic Visitation

Eucharistic visitation offers the opportunity to share many blessings through prayer and the bringing of sacramental presence of our Lord and Savior, Jesus Christ, to those members of our parish who cannot, for whatever reason, come to church at appointed service times.  Visits may also include healing prayer, if desired.

"Eucharistic Visitor" is the term used at Grace church currently as "minister" holds a very special place in Anglo-Catholicism.  Only a priest many bless, consecrate (make holy and set apart) absolve or anoint. Eucharistic visitors may lay on hands, pray (intercede) and ask for healing.

Sacrament (defined in the Catechism - p. 857 BCP); is an outward and visible sign ( bread, wine, and water) that conveys the inward and spiritual grace of Christ's presence.  The priest administers and consecrates the sacrament (BCP p. 856) and the Eucharistic visitor brings the grace of God.

There is spiritual communion available to those who are unable to receive the elements.
